We have Certified Application Counselors (CACs) on our team. They help you and your family know your health insurance options, like Medicaid, Medicare, or health insurance exchange applications.
Affordable Care
If you make less than 200% of the Federal Poverty Level Guidelines, for example around $53,000 or less for a household of four, we will lower your cost. This depends on the number of people in your house and how much money you make. No one will be turned away because they can’t pay a bill.

Do you take my insurance?
We accept Medicare, Medicaid, Medicaid Managed-Care like Buckeye, Paramount, Molina, CareSource, and most commercial insurance.
What if my bill is still too high?
No one will be turned away for not being able to pay. We offer payment plans and other options. Call Health Partners to talk about a discount or to set up a payment plan.
I don’t have insurance. How much does it cost?
Patients pay based on what they can afford. Our team can explain the Sliding Fee Scale and help you apply to receive a discount. We can also help you find out what kind of insurance you may be eligible to apply for, including Medicare and the Insurance Marketplace.
