School-Based Vision Program
We are helping to close achievement gaps! Children need to see to read and learn. Students that receive glasses improve their grades and engagement in the classroom. Health Partners’ School-Based Vision Program provides these services to students at no cost:
- Vision screening
- Eye exams
- Glasses

How do I sign up for the School-Based Vision Program?
Sign up by filling out a consent form and turning it in to your school. Consent forms are sent home from you child's school.
