Wellness and Preventive Care

Our health care teams work together. They help you get and stay healthy. They also share ideas for making healthier choices. Things we do:

Short-Term Sick Care

When you get sick, our goal is to help you get better fast. We help with things like:

  • Sore throats

  • Ear aches

  • Muscle sprains and strains

  • Headaches

  • Other common illnesses

Long-Term Disease Management

It can be hard to know where to start when you find out you have a disease. We help you understand the disease. We teach you ways to manage it. We can help you with:

  • Diabetes

  • Asthma

  • High Blood Pressure (Hypertension)

  • Hepatitis C

  • High Cholesterol

  • Lung Disease


Women’s Health Services

We help women get the care they need. We do regular pap smears, pelvic and breast exams, and Sexually Transmitted Disease (STD) checks. We offer family planning options, including birth control pills, Depo-Provera shots, IUDs, and implants.

Laboratory Services

Labs run tests on bodily samples (like blood and urine) to help find out what’s happening inside of your body. You can get all of your lab services done at Health Partners. You do not need to call ahead.

We partner with a local lab to make these services possible. This means the lab, not Health Partners, will directly send you a bill. A sliding fee scale is available to help keep costs low.

OB Services - (Obstetrics/Childbirth)

Our Kenton Community Health Center is the only location with care for pregnant women. Call (419) 679-5994 to make an appointment or ask questions.

Early pregnancy care is important for the mom and baby’s health. We encourage any woman who thinks she may be pregnant to schedule a visit as soon as possible. We offer full services for the entire pregnancy. You can get care from when you first find out, through the delivery of the baby, and even after the baby is born. Our care teams will see both the mom and new baby.


Hepatitis C Care

Care for Hepatitis C isn't what it use to be. In most cases, it's fully treatable with medicine. Health Partners can provide the care you need at a cost you can afford. You deserve to live well. Call us to set up your first appointment.
