Regional Health Center Manager

The Regional Health Center Manager provides leadership and direction to assigned Center Directors for the day-to-day management of Health Center activities at East Toledo Community Health Center, Bryan Community Health Center, Defiance Community Health Center and Tiffin Community Health Center. Consults with and advises the C.O.O. on problems relating to the operation of the health center facilities. Responsible for overseeing overall integrated health operations, including: medical, dental, chiropractic clinical support, behavioral health, and clinical quality improvement at sites as assigned. The Center Receptionist/Referral Specialist provides first and last impressions of the organization so they must be customer-focused and professional in carrying out responsibilities. This position is responsible for answering all incoming calls, greeting callers, providing information, transferring calls, scheduling appointments and taking messages. This position will also be responsible for facilitating and managing patient referrals between all HPWO providers/services and outside specialists/agencies. This includes facilitating and managing the summary of care received from the outside specialist/agency, therefore completing the coordination of care for that outside service. This process supports HPWO as the patient’s primary care provider and medical home who assumes responsibility of care for the services rendered at outside specialists/agencies.

We currently have one Behavioral Health Provider position at our school-based Spartan Health Center in Lima, Ohio. This individual will work in collaboration with our medical providers to provide integrated medical and behavioral health services to patients in the Lima City Schools. Interventions will be brief, solution-focused treatment that is effective and consistent with a population-based model of care.
