Activities will vary based on patient need, but basic functions will include helping patients to navigate the healthcare system, understand and be connected to Health Partners’ services and community resources as well as help to eliminate barriers to care and reduce the cost of care.

The Referral Specialist/Medical Assistant is responsible for facilitating and managing patient referrals between all HPWO providers/services and outside specialists/agencies. This includes facilitating and managing the summary of care received from the outside specialist/agency, therefore completing the coordination of care for that outside service. This process supports HPWO as the patient’s primary care provider and medical home who assumes responsibility of care for the services rendered at outside specialists/agencies. Health Partners of Western Ohio is a Federally Qualified Health Center system established in 2003. We are looking for energetic, committed, and mission-driven individuals! We currently have one Behavioral Health Provider position at our East Toledo Community Health Center in Toledo, Ohio. This individual will work in collaboration with our pediatric providers to provide integrated medical, behavioral health, dental and pharmacy services to patients. Interventions will be brief, solution-focused treatment that is effective and consistent with a population-based model of care.
