This position is responsible for providing education and assistance to health center patients through supporting health educators. Health Coach/Community Outreach Worker will demonstrate and promote the values of respect, honesty, care, and dignity for all individuals. The individual will be responsible for assisting patients who have unmet, complex, or ongoing medical needs with a variety of care coordination activities. Activities will vary based on patient need, but basic functions will include helping patients to navigate the healthcare system, understand and be connected to Health Partners’ services and community resources as well as help to eliminate barriers to care and reduce the cost of care.
